What is Toe Walking?
Toe walking refers to when a child walks on the balls of their feet with their heels off the ground. It’s common for toddlers to walk on their toes as they learn to walk. In most cases, children naturally begin walking heel-to-toe by around 2 years of age. However, if toe walking continues beyond that or develops suddenly, it may be time for an evaluation.
Is Toe Walking Normal?
Occasional toe walking is a normal part of early development as children are first learning how to walk. Many children outgrow it without treatment. However, persistent toe walking can sometimes lead to muscle tightness or indicate an underlying condition.
Common Causes of Toe Walking
Several factors can contribute to toe walking, including:
- Habitual (Idiopathic) Toe Walking (ITW): The most common cause of toe walking, where there is no underlying medical condition, but it typically occurs out of habit or is a preferred way of moving for a child.
- Tight Achilles Tendon or Calf Muscles: Tight calf muscles or Achilles tendons can make it difficult for a child to place their heels on the ground.
- Foot Structure: Flat feet or other biomechanical issues may affect the way a child walks.
- Neurological Conditions: In some cases, toe walking may be associated with developmental or neuromuscular disorders.
When Should You Be Concerned?
Schedule an evaluation if your child:
- Continues toe walking after age 2
- Cannot comfortably walk flat-footed
- Complains of foot, ankle, or calf pain
- Frequently trips or has balance issues
- Suddenly starts toe walking after previously walking normally
Early evaluation can help identify the cause and prevent future problems.
How Is Toe Walking Treated?
Treatment depends on the underlying cause and may include:
- Stretching exercises
- Physical therapy
- Supportive footwear
- Custom orthotics
- Bracing in some cases
Surgery is rarely necessary and is typically reserved for severe cases that don’t respond to conservative treatment.
